I can understand your concern regarding to your son's health. The high level of TSH is suggestive for under active thyroid or hypothyrodism. To decide rather to treat it or not, he needs to check also the FT3 and FT4 level and anti TPO and anti Tg antibodies. Usually it has an autoimmune origin but it should be confirmed. Gynecomastia in these ages in most of the cases is transitory and after puberty it will resolve without treatment. He should try to lose weight for an earlier improvement.
