What Could Cause Severe Pain In Abdomen, Diarrhea And Strained Mid Back?

I have had severe abdominal pain for a week. It started as severe gnawing pain just below my xyphoid process localized, nonradiating. Lap chole in 1998. Nausea only due to pain. Normal Bowel movements initially. Went to local ED last Wed and last Fri and all tests were normal (Ct with and without contrast), pregnancy test, amylase, lipase, lactic acid, CBC. Have been on protonix for a month. Been on carafate four times per day since Sat. Now having diarrhea multiple times per day and pain is more colicky and generalized all over abd. Right mid back feels strained a little!! I cN barely take this!!

General Surgeon 's  Response
Hi,

Sever abdominal pain in upper part followed by diarrhea and colicky pain can be most probably due to colitis related problems. Now there is accompanying pain too.
I would advise you the following in such a situation:
Consult your Doctor again for clinical re-evaluation and examination.
Investigations of stool and blood may help to get an exact cause.
If there is any intestinal infection a course of an antibiotic, metronidazole may help.
Also take a symptomatic treatment with antispasmodic.
Continue Carafate and take a PPI like Omeprazole.
If there is still no relief then you may need further evaluation with CT scan of abdomen and colonoscopy along with Upper GI Endoscopy.
Also consult an Orthopedic Surgeon if the treatment of abdominal conditions does not relieve your mid back strain and get treated accordingly.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant, General Surgeon
