Does Taking Coumadin Cause Loss Of Blood Through Stools?

Dear Dr. My mother has been on coumadin for several years due to two attacks of DVT in her legs. Now (yesterday morning) she was admitted to the hospital for internal bleeding during an outbreak of diarrhea. For the diarrhea her physician put her (Mon) on a liq-only diet of Pedialyte for 24 hours followed by a BRAT diet for 24 hors. The morning after the BRAT diet finished (yesterday) she lost a lot of blood in her stool and was sent to the ER for treatment. They did an INR and found it was 7.9. Her normal range is 2 to 3 on the coumadin dose she is taking (3 mg/d, 6d/wk). I am very concerned that the bleeding was CASUED BY the diets of the last two days. She ate no vegetables after Mon afternoon and took no supplements except Vit D3 (1000 u/day) and CaCO3 (liq. 1000 mg). Is it possible that she is suffering from coumadin poisoning?

Hi,

Definitely, internal bleeding in the body may be due to overdose of coumadin. As your INR is 7.9. It is too high & coumadin overdose seems a reason for it. The dosage of coumadin is to be controlled according to the level of INR. Normally INR in a person should be 0.8 to 1.1. In patients on coumadin, the level should be 2.0 to 3.5. As your INR is high, your coumadin dose should be lowered under guidance of your doctor, till INR comes to 3.0 or so. If the bleeding does not stop or becomes more, then treatment by hemostatic under guidance of your treating is urgently required.
