General Surgeon 's  Response
Hello,
I can understand your concern for your symptoms.
1) Peri-anal pain associated with bowel movement is due to anal fissure.
2) This is a condition in which small linear tear develop on inner wall of lower part of rectum.
3) Generally constipation is aggravating factor for this condition.
4) You can use local application of anesthetic agent ointment for this pain, anti-inflammatory group of medication to reduce swelling in anal region associated with fissure.

Hope I answered your query. If you have any further doubts, I would be happy to help you.
Have a nice day!
