Suggest Treatment For Painless Sex

Hi, i'm 33yrs. I've always had painful sex(started having sex 6yrs ago). Vaginal swab showed i had infection for which I've been haven different antibiotics. 2 months ago, i started having pains once there is any pressure on my stomach and periodic sharp lower abdominal pains. The 1st scan showed nothing abnormal. A 2nd scan showed considerable fluid in the pouch of douglas. I was asked to a HVS. This showed no yeast, bacteria or T.vaginalis growth, epithelia cells: ++/HPF, Pus bells: 1-3/HPF at 37C after 48hrs incubation. What does this mean? What do i do to stop the pains permanently?

OBGYN 's  Response
Hello,
I have gone through the query and understood your concern. Pain during intercourse or 'dyspareunia', can be caused due to several causes and infection of the cervix and/or the vagina is one of them. The other possible causes are a retroverted uterus where the ovaries are prolapsed into the pouch of Douglas and pain is experienced on contact, cysts of the ovary, adnexal masses, polyps of the uterus and cervix, which often bleed etc. Appropriate management can be initiated after a complete appraisal. The finding of fluid in the pouch of Douglas in the sonogram indicates ovulation. As of now, there is no evidence of any infection. The occurrence of epithelial cells in abundance needs further evaluation. Hope this helps.
