If such patient comes to me, I would examine the oral cavity and posterior pharyngeal wall thoroughly and request to have warm saline gurgle four times daily, an anti inflammatory drug, anaesthetic syrup and a course of antibiotic of at least 7 days. You have started antibiotic 4 days ago. It would be wiser to wait a few more and following the rest of remedial measures.
A non improved case after antibiotic course may need the endoscopic procedure in future.
