General Surgeon 's  Response
Hi,

The commonest causes of pain in anus without any fissure or so can be one of the following:
Ano-procitiis wherein there is a swelling and inflammation in the anal and/or rectal area.
Cellulitis, the first stage of an abscess.
Anal sphincter spasm.
This can be found out by the following ways.
Consult a General Surgeon or a Proctologist for per-rectal digital palpation and ano-proctoscopy.
MRI of the affected area gives signals if there is any inflammatory lesion.
Till then I would advise you the following:
Sitz bath in a tub with warm water for local soothing action.
Continue Naproxen and stool softener.
Further management as per the findings.
Avoid stress and anxiety as this can be one of the reasons.
Get stool and blood test to see for any infection.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant, General Surgeon
