Hi,

Thank you for your query. I can understand your concerns.
Your diabetes is uncontrolled. Ideal blood sugar should be Fasting-100 mg/dl and post-prandial (PP)-140 mg/dl and HbA1C You need to take sulfonylurea e.g. glimiperide 1mg before breakfast and to increase the dose of glycomet to 500 mg 3 times a day. Check blood sugar again after a period of 7 days.
You have to follow diabetic diet and to avoid extra salt & salty foods like Potato chips, cheese, peanut butter, salted butter, pappads, salted peanuts, salted cashew/almond,soups and salt preserved foods -pickles and canned foods (fish)
Do aerobic physical exercise for 20-30 ′ at least 5 days per week.
Alcohol moderation-abstinence –preferred; For diabetes you can take also one teaspoonful of methi (fenugreek) to be soaked in a cup of water over night and to be taken orally in the morning.

dear sir I can understand your concerns
in my opinion your fasting blood sugar is under Controll but your post breakfast blood sugar is slightly more is I would like you to increase your exersise and fine tune your diet pattern
in case your are already doing diet and exersise well I would like to suggest some changes in your medication
it worth considering of taking new medications like DPP4 inhibitors which will protect your pancreas from damage and also control your blood sugar
it's avilable as Januvia 100 mg to be taken once daily in bed time
consider speaking with your doc for treatment changes
regards
