Pregnant. Blood And Leukocytes In Urine. No Painful Urination. Have Back Pain. UTI Without Nitrites?

Hi, I am a 29 year old female, 29 weeks pregnant. I have no chronic health issues. This is my 6th baby. My BP is normal, so is the CBC I had last week. 2 hr pp glucose is normal. Last week I noticed trace amounts of blood in my urine when I test with the chemstrip 10 tests. Two days ago there is a large amount of blood in my urine as well as a small amount of leukocytes. It does not hurt when I pee, but I have strange back pain that I have never had before. Can this be a UTI without the presence of Nitrites? Or could I be looking at something different? What is the best plan of action?- I have limited health insurance:(

Reviewing the above symptoms, I think we need to be concerned of a kidney stone. They are generally more common during pregnancy as a result of the relaxing effects of progesterone. Back pain, blood in urine and leucocytes without any germs are very suggestive, If this be the case, then the entire urine is going to be stained with blood.

It could still be a UTI. Not all UTI present with nitrites on dipstick. It is typical is the UTI is caused by a gram negative bacilli which are the most comon causes. If the UTI is caused by another germ, you will not have this. UTI are also more common during pregnancy.

For now, i will suggest you get consulted, do an abdominal ultrasound, a urine culture. This will make the picture clearer. You may also take some pain medications and increase oral fluids intake. It is of great help in either case.
