Numb Toe, Tingling In Lower Leg, Pain In Hip. Had Stumbled While Getting Up. Suggestions?

I am a 50 year old women 3 days ago I stumbled as I got up from a crouching position, I felt something "go" in my right hip. Just thought I had pulled something I've tried to carry on, put its just getting worse. However I can not sleep as the pain is so intense when I lie or sit and I am more comfortable standing. My big toe is numb and I have tingling in the outer side of my lower leg. The pain in my hip area is at the top of my thigh, through to my groin and at times down to my knee.
I just have an overwhelming feeling of not knowning what to do with myself and at times its reduced me to tears the pain is so bad.

Hi,
Thanks for writing in.

You report of a sudden fall 3 days back and possible injury to your right hip resulting from it.
There are few small bones in the lower back region which form the lumbar spine and scrum bones. These bones are aligned one on top of the other and have soft discs between them. After a fall, there may be some malalignment of the these bones with one bone displaced forwards or backwards with respect to another bone. This is called spondylolisthesis. There is a remote possibility of a fracture of these small bones too.

A X ray will reveal any fracture or spondylolisthesis. At the same time, there are nerve roots exiting the spinal cord which are passing in between these small bones. These nerve roots can get pinched after a fracture, spondylolisthesis or disc disease and cause sciatica. In many patients with this problem, they will be comfortable standing but will get a sharp pain while sitting of lying down (when the spine is flexed). This is probably happening to you. If the pain is very severe, your doctor may ask for a MRI scan of the lumbar spine to see for any serious nerve root pinching.

The pain in the hip joint might occur from injury to the cartilage in the hip joint region. If pain is severe a X ray for hip joints might also be done. Please also get tested for osteoporosis if you have generalized pain involving bones and joints.

You can take non steroidal anti inflammatory drugs (NSAIDS) for some time. If there is severe pain and difficulty in doing daily tasks please consult your orthopedic surgeon at the earliest.

Hope this helps.

Psychiatrist Dr. Anjana Rao Kavoor's  Response
Hi,

You report history of injury to your hip few days ago, with a feeling of something giving way in your hip. Currently you experience intense pain, tingling and numbness in your leg which are signs of a nerve compression. It is possible that a slip disc or a fracture caused by the fall is compressing on a nerve leading to the symptoms.

It is advisable to visit an orthopedician at the earliest, You may expect the doctor to ask for an x ray or a MRI of the affected area to confirm any injury. The treatment would depend on the extent of injury.

Hope this helps,
Dr A Rao
