Is Nonweight Bear With Gradual Increase The Right Treatment For Bilateral Patellar Contusions And Tibial Plateau Fracture?
I am a family practice physician in Maryland. My 30-year-old niece was struck by an automobile two weeks ago. She sustained bilateral patellar contusions and a tibial plateau fracture. She s been advised to nonweight-bear with gradual increase. She lives in Arcata, California, they have somewhat limited orthopedics at their local hospital. I have encouraged her to get a second opinion just to make sure that she s following the right course of treatment. Could you suggest anyone that s good but conservative, she is currently staying at her mothers house in Vancouver because she has two children under three, which is difficult to manage with nonweightbearing orders. Any help would be appreciated. David G Freas M.D. (cell number is 301-704-1037).
Hi If tibial plateau fracture is stable and not displaced then a plaster cast for at least 4to 6wk is advisable with follow up x rays and then partial weight bearing should be done along with walker. If you do fiber cast plaster then you can put weight on it. Get well soon. Thank you.
