Hello,

If the pain is related to ovulation and it is advised surgery due to failing in all other treatments them ovaries should be removed.
Both ovaries and the uterus too should be removed because ovulation happens in ovaries. I can suggest you rethink the conservative treatment with estroprogestative contraception pills which stop ovaries from ovulation.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Olgeta Xhufka, General & Family Physician
