How To Safely Wean Off Isosorbid Mononitrate?

I am so very tired and a new medicine that i have been taking since nov 3 after a small heart attack is isosorbid mononitrate cr 30 mg xr24h. I am also on plavix, magnesium, 81 mg aspirin, carvediolol 6.25 mg x 2, and crestor, plus 15 mg prednisone. how can i safely wean myself off the nitrate
