How Long Does Medication For Thrush Take To Be Effective In Children?

My son is a year and a half and he is about 21 pounds. He has had a fever on and off for about 4 days now. I took him to the ER and they said he has thrush. I've been giving him the medicine for a little over 24 hours. The medicine is Nystatin or something like that. He take a teaspoon every 3 hours. But he still seems miserable and can't close his mouth because it still hurts too bad. About how long does it take for the medicine to work and help wit the pain?
