Fever is almost a week duration so better to investigate with compete blood picture, for malarial parasite, for typhoid fever and complete urine examination and if suggestive xray of chest and blood cultures. Depending on reports and child's general condition hospitalisation or OP basis treatment can be decided.
Hope your child recovers soon
Take care
