Experiencing Pain Behind The Knees

We live overseas. My 3-year old daughter has had a fever for 6 days now, and her cough is worsening. She was lethargic the first day, but overall seems ok during the days. Her fever seems to spike at night. This morning she woke up with severe pain behind her knees. Is there a risk this could be rheumatic fever? What tests should the local doctors be running?

Three-year-old child with 6 days fever - what are you doing?? Take her ASAP to nearby hospital for examination chest X-ray and blood counts. Let pediatrician see her and advise accordingly.
