Ejection fraction of 40% is not bad. Patients with such ejection fraction may live for years. If he has no heart block and no arrhythmia, then there is no need for pacemaker. Your father's main problem is blocked vessels and angina. Everything depends on the size of blocked vessel and the degree. Of it is small vessel, it may not be dangerous. Anyway he should receive aggressive treatment for angina and atherosclerosis. The prognosis depend also how he will respond the treatment. Sometimes angina disappears in 6 months, so patients have no heart attack for a long time
Take care
