Child Has Fever, Lethargic, Cold Feet. Time To See A Doctor?

Hi, my 20 months old toddler has had a fever (39 Celsius) for the past three days. She is very lethargic and just sleeps all the time. Despite her fever she has cold feet . This morning she seemed more perky for a couple of hours but she has since gone back to just lying down and starring or sleeping. Is it time to see a doctor? Thanks for your advice! Ky
