fever not responding to antibiotic and antipyretic , that too with shivering, it is always better to exclude malaria and urinary tract infection
you may try paracetamol 15mg/kg body wt every 6 hours along with alkaline citrate 2.5ml diluted in water 2 times daily
you may try chloroquine syrup emperically with the advice of your doctor, but better consult a pediatrician

A baby having fever for one week should be investigated for the cause of the fever.

Does he have any associated complaints like cough, cold or burning urination or increased frequency of urination etc?

Investigations like CBC, CRP, Peripheral smear for malaria, urine examination may be ordered by your doctor after physical examination.

i can feel your concerns

you can use paracetamol and ibuprofen for fever alternatively

use these medicines 6 hourly

tapid water sponging also helpful

i advise you to consult your doctor and have a detailed physical examination

it is important to find focus of infection

she may need a few investigations and blood tests

avoid cold and soft drinks

give her plenty of juices and fluids

i advise to complete course of antibiotic
