She is confused not really talking a lot. She is refusing care at this time. Are you able to assist me in some advise to tell her to try to convince her to get care and if not is there anything we can do to help her?

Psychiatrist 's  Response
This is an medical emergency

The bleeding can be profuse leading to shock and other complications. Not necessarily that the bleeding will be visible. Extended abdomen can denote to portal hypertension, the symptom of which is bleeding during bowel movements

You should call emergency ambulance and get her admitted even if she is not cooperating

Good luck

General & Family Physician Dr. Chobufo Ditah's  Response
Hi and soorry to hear about what your mother is going through.

From the information you have provided, your mother probably has liver failure and has developed hepatic encephalopathy. She needs to get evaluated and most likely will need a liver transplant in the long run. For now, palliative measures may improve her quality of life.

If there are no options to get her treated with probable liver transplant, then I understand her frustration dealing with a terminal illness. The best thing is to tell her how much she means to you and the pain it will cost you see her die. Tell her how much you need her for your life to be complete and your total commitment to seeing her strong or at least living good. Refusal of treatment is often common and getting a solution to this is difficult. Keep trying as any times as possible and she may just change her mind.

Hope you find this helpful.
Wish you success with this and your mom the best outcome her condition permits.
