Why Do I Feel Itching And Tingling All Over Especially At Night While Trying To Sleep Apart From Peripheral Neuropathy?
Question: I just had a new patient come in with a long list of problems. The main problem that is causing problems now is a peripheral neuropathy she believes is from Lyme or Epstein-barr (both of which tested positive -IgM+ within the last month). Her s/s are "insane itching, tingling all over especially at night while trying to sleep". She complains of seizures and tremors of her hands and feet. All of her symptoms started at the age of 46 four years ago when she came down with Mono and then two weeks later a doctor gave her a shingles vaccine. One month later she developed a rash on her legs and back and these painful neuropathy symptoms began. Her positive labs include an elevated VEGF of 396 (31-86), Positive EBV VCA, IgM, IgG, EBNA; decreqsed C3a desarg fragment, normal C4a 1438 (0-2830) elevated T/AT complexes 7.4 (less than 4.3 is nl)
low estrogen/testosterone. MRI of brain a few small foci of increased T2 weighted /FLAIR signal intensity in periventricular deep white matter consistent with lyme inflammation and a decrease in blood flow in the watershe areas of the frontal lobes and temporal lobes on SPECT scan/

Brief Answer:
She is having very severe lyme s disease.
Detailed Answer:
Hi,
She is mostly having severe form of Lyme's disease. Brain involvement in Lyme is something uncommon and considered as severe form of infection. She should also test for vitamin B12 level because deficiency of vitamin B 12 can lead to tremors, tingling etc.
She may need vitamin b 12 supplements if Deficiency is there. Another useful drug in her case would be pregabalin or gabapentin. These are specifically for neuropathic symptoms like tingling. Also rule out diabetes if not done previously. Please let me know if patient is smoker.
I will be happy to help you further. Wishing good health to your patient. Thanks.
Regards
