What Does This Semen Analysis Report Indicate?

Question: Hi Doctor

We are trying for baby since 1.6 years. We have done 3 IUI, however all negetive.I did my semen analysis and the result is as below:

Sperm Count.   : 76 million

Sperm Volume : 1.5 ml

Sperm Motililty : 60 to 70 %

Rapid moving   : 30 to 40%

Viscocity           : High   40 min

Please find the attached report for your review.

My wife has PCOS and is undergoing a treatment for the same. Now doctor said that it is under control and she is having regular periods (28 days cycle). She is under medication for pcos and also folic acid tablets since last 1 year. Iam also taking tablet Popson since last 5 months.
I have below questions :

1.What may be the reason for not having child.

2. What is the reason and  treatment for high vicosity. How long it would take to get cured completely.

3.How long should I abstain before IUI. Last time i abstained for 7 days for better result, however the motility and count was low as compared to the current report.

4.Can i collect sperm in the same container for the second time within 10 minutes of gap .Would it have any negetive effect in the sperms.

Semen analysis is fine. There can be other reasons for not conceiving.

Detailed Answer:
Hi, I think your semen analysis is fine. Report is not attached. Please upload it. Regarding the viscosity, it can be high in case of infections. So, get a semen culture done. You can take antibiotics like doxycycline empirically also. Discuss with your doctor regarding this. It may take 10 to 14 days to clear up the infection. If IUI is being done, viscosity is not a major issue. Before IUI, 3 to 5 days abstinence is fine. 7 days is too long, it decreases motility and count also. There is no need to collect two semen samples 10 minutes apart. Even if you want to do so, collect in different containers. I think you can take some antioxidants for 3 months to improve the count and motility after processing of semen. You can try 3 more cycles of IUI. Regarding your wife, get a thyroid profile and prolactin levels done for her. Also, a day 2 FSH, LH and AMH level should be done. IF Value of LH and AMH are high, may be she requires a laparascopic ovarian drilling. It will increase chances of getting pregnant naturally also. So, finally the chance of getting pregnant in one cycle of IUI is only 10 to 15 percent even if everything is fine. So, you need to try atleast 6 cycles of IUI before losing hope. If it doesn't work, better to go for IVF. Hope I have answered your question.

Hi, your semen analysis is normal. Viscosity is on a higher side, but if liquefaction time is less than 60 minutes, it is not a major issue. Do a semen culture as I told you earlier just to rule out any infection. You can take antibiotic doxycycline for 10 days empirically. Talk to your doctor regarding this.
