What Does This Blood Report Indicate?

Question: my friend was experiencing chest pains, went to the hospital and the results of a blood test were as follows:

Cardiac profile: CPK 348 u/l ref range (38-174)
CKMB 21 u/l ref range ( 7- 25)

Immunology :
Troponine T 0.008 ng/ml ref range (0.127 - 0.249)

what does this mean??

As per your query,They are all cardiac enzymes.Cardiac enzymes are present inside the muscles of the heart,When there is a myocardial infarction(heart attack),Cardiac muscles are killed(necrosed) due to lack of blood supply,Hence the enzymes present in them are released into the main blood stream,an estimate of these enzymes help us to differenciate between unstable angina and myocardial infarction.

CPK and CPK MB are highly unspecific for cardiac tissue injury and raised serum level is seen in a number of condition like skeletal muscle injury,neuronal injury ,Hence an increase is their level does not have any significance according to the latest trials.

Trop I and Trop T are the most specific enzyme to rule out cardiac ischemia and myocardial infarction at present.Your trop t levels are Below the normal ,Hence likelihood of myocardial infarction are very low,However the pain can be because of unstable angina.A thorough cardiac investigation should be done.

As per you question this means that it was not a heart attack!(Your trop and CPKmb both are normal) However;A stress thallium should be done to rule out coronary artery disease.

Advice
Repeat trop T after 6 hours
Stress thallium.

i did not mention this before but an ecg was also done which the doctor said was normal.... my friend also suffers with back pain and insomnia could these three conditions be related in any way?? all these conditions are ongoing and not one offs..

As ecg and cardiac enzymes are normal it was definitely not a heart attack .
Yes back pain can be because of cervical spondylosis.Cervical spondylosis can be a cause of chest pain.Insomnia leads to anxiety and anxiety can cause a panic attach ,Which mimics chest pain.All of them are linked.Initially thorough investigations should be done to ruke out coronary artery diseases,After that treatment should be given for insomnia and back pain.

My friend is in the construction industry, should a mild exercise routine be started to help with symptoms before consultation with a doctor? If so what form of exercise would be suggested?

30 minutes of brisk walking should be done everyday.
If possible moderate exercise should be done three times a week atleast.
Rest a thorough investigation should be done as early as possible to rule out the cause if the problem.
A low dose anti anxiety medicine like clonazepam can be taken for few weeks till tests are done.
