Posted on Mon, 30 Jun 2014
Question: I had a CT scan show an adrenal mass while I was in Afghanistan. Upon return I have done some research on the adrenal glands, etc. Now that I think about it, I have had problems with a LOT of the symptoms present in endocrine diseases. How can I approach my primary care physician to get the specialized care I need? I am active duty military and am scared this may be overlooked.

Brief Answer:
You need proper clinical exam & investigations.

Hello XXXXXXX
Thanks for writing to HCM

It is important to know your symptoms.
Adrenal mass be the incidental finding.Adrenal incidentaloma is an adrenal tumor found by coincidence without clinical symptoms or suspicion. You need proper clinical examination.

Adrenal mass can be benign and malignant neoplasms. It is important to look for symptoms.
It is important to determine that mass is biochemically active or not.Tests are needed to determine the level of dexamethasone suppression,24-hour urinary free cortisol, urinary 17-hydroxycortisol and 17-ketosteroid,plasma androgens, testosterone, androstenedione, renin, aldosterone, urinary vanillylmandelic acid, plasma catecholamines and plasma metanephrine and normetanephrine.

Nonfunctioning mass <3 cm should be followed up with CT scans or MRI.If the mass has not grown at the 3 month or 1 year follow up then no further testing is required.

Some of the mentioned symptoms are related to hormonal imbalance. It is important to rule out that symptoms are due to adrenal mass.

Based on clinical assessment, she may need some blood and urine tests.
At present, she need follow up scan after clinical assessment and investigations.
FNAC can be done for confirming the nature of lesion.Treatment depend upon findings.
