What Does The Following Ultrasound Report Indicate?

Question: Hi,

My primary care physician suggested me to get an ultrasound done because i had some changes in my bleeding pattern during my periods ever since i discontinued birth control.

Ultrasound Findings: The uterus is normal measuring 11*3.7*5.9 cm in sagittal, AP and transverse dimensions respectively. The uterus is lobulated and contains multiple myometrial masses, the largest of which is located posteriorly to the left of midline measuring up to 3.9cm. This distorts and displaces the adjacent endometrium. Another located posteriorly in the artery of the uterus measures upto 2.3cm and contacts but does not deform the posterior margin of the endometrium. Another located anteriorly in the fundus is subserosal in location and measures up to 2.3cm. The endometrial cavity is empty. The endometrium, measures up to 2.3cm. There is no free fluid present within the cul-de-sac.

The right ovary measures 4.1*2.4*2.1 cm and left measures 4.3*2.7*2.5cm. Both ovaries contain follicles. A simple cyst in the left ovary measures up to 3.2cm. The adnexa are clear.

CONCLUSION:
ENLARGED UTERUS CONTAINING MULTIPLE LEIOMYOMATA THE LARGEST OF WHICH IS LOCATED SUPERIORLY IN THE LEFT SIDE OF THE FUNDUS AND IS SUBMUCOSAL IN LOCATION DEFORMING AND DISPLACING THE ENDOMETRIUM. A SMALLER POSTERIOR LEIOMYOMA ALSO CONTACTS BUT DOES NOT DEFORM THE ENDOMETRIUM.

3.2 CM SIMPLE CYST IN THE LEFT OVARY.


Questions:

1. How should this conditions be treated?
2. I am actively trying to conceive a baby? I am 28 yrs old with no previous preganacies. I have regular periods every month but i see brown discharge in the middle of the month (13, 14, 15 days)
So is this normal?

3. Do I need surgery?

Thanks so much for your response. Just had some follow up questions

1. Is myomectomy used to treat both fibroids as well as the cyst?

2. Is it a surgical procedure? Typically wats the recovery timeline after surgey on this?

3. How long does the surgical procedure take? will it leave any big scars on the body?

Hi,
Sorry for the very long delay in my response.
Here are the answer of your questions.
Myomectomy is a surgical procedure to remove fibroid.
It can be done through hysteroscope,laproscope and it can be laparotomy depending upon fibroid position.
Scar is more with laprotomy.
The above two mentioned procedures are day care procedure( old procedure) and in laparotomy it is 4-5 days of stay.
Recovery is 2-4 weeks usually but 4-6 weeks after laparotomy.
