What Does The Rorschach Test Report Indicative Of?

default
Posted on Mon, 12 Jan 2015
Question: I am a Male, born in 1980, I would like to give some background here before asking my questions. I was referred to a clinical psychologist in 1999 for evaluation, he performed a Rorschach test. The test report is attached, on the basis of the report a psychiatrist has me on medication since. During initial years of the treatment I was taking upto 20-24 pills a day of 6 or 7 different types. Since I have been on medication I have gained 40 Kgs of weight going from 75Kg (5 ft 8in) to 115Kg(5 ft 8in). I have taken opinions from a couple of doctors but they also wrote prescriptions with the same medicines from other manufacturers. What I would like to ask is:
What am I suffering from? Do I need to take medication for my entire life? Should I even be thinking ever about marriage and kids? Is the problem genetic?

Brief Answer:
Please provide details of symptoms, medicines and relevant history

Detailed Answer:
Hello Mr. XXXXXXX

I read your query and also seen reports.

Rorschach test is done when there is difficulty in differentiation between psychotic and anxiety features.Your test is suggestive of anxiety features. However with this information I can not comment about your diagnosis.
It may be anxiety spectrum disorder, depressive disorder or obsessive compulsive spectrum disorder.

I need to know," what symptoms did you have at onset as well as later on after treatment and currently which symptoms are still there?"This will help me to comment about your diagnosis.

I also want to know, "which medicines have you been given?" If feasible upload prescriptions or let me know names with dose and at which time period ?

In addiction, please tell me something about your education, family and occupation and if you have any stress- social, economical, emotional etc.

After getting above information , I can guide you about your possible diagnosis, which medicines you should take and probably for how long.Comments on marriage and kids also can be given after that.

Waiting for your reply to help you further.

Brief Answer:
You have anxiety disorder likely,it can be treated

Detailed Answer:
Hello Mr. XXXX,

Thanks for information.

I have seen your all attachments.
They includes symptoms of anxiety features.
Most of the medicines mentioned are for anxiety disorder.
Sertraline, inderal and clonazepam are the medicines for anxiety disorder.
Rizadin 10 mg is an old antipsychotic drug. It can be used in anxiety problem. Rozidol is risperidone- an atypical antipsychotic medicine. your doctor might have suspected some psychotic features.

For confirmation of anxiety disorder diagnosis, detail history and mental status examination are required by a psychiatrist. Frankly speaking you need to consult some another psychiatrist to reassess your diagnosis non-judgmentally.

If it is confirmed anxiety disorder, it can be completely cured with pharmacotherapy as well as psychotherapy.
You mostly do not need to take medicines for lifetime.
In my opinion you have not been given sufficient dose of needed medicines to treat your problem.
Simultaneous application of psychotherapy-Cognitive Behavior Therapy - CBT would fasten the recovery.
You can go for marriage and have kids depending upon your cognition and occupational health.
This problem is caused by biological, social and environmental factors. It is not purely genetic.

Hope I have answered your concern.

Regards,
Dr.Chintan Solanki.

Please find below the requested details:

Symptoms: Since childhood my nature has been like really quite, introvert, shy and kept to myself. While growing up I was really scared of dad, but he got paralyzed when I was 10 years old and then I felt there was no one I could turn to for help when other kids picked on me and my brothers. Lot of anger was building up at this stage inside of me, but I kept it bottled up. Finally I exploded after 12th class practical exams but before theory exams. I had a sinking feeling in my chest that I was not able to prepare properly in fact I had given up on it and knew that I would fail so I refused to sit in the exams. Then for two whole years I did not step out of my house and I use to sleep all day and watch TV all night and not talk to anyone. During this time my dad started acting up since mom arranged for a settlement of a case so that we would get some money. My dad took control of the money and started acting like a jerk. which led to fights and me and my younger brothers stood by mom. Things escalated and we ended up in court, IG office, etc etc. I became really impulsive, verbally abusive worse than a sailor, physically aggressive. But at the same time i got back on track with education and and also we moved out of the house away from my dad and then the medication started to have a soothing effect and I no longer reacted all that impulsively and aggressively, my behaviour had become much more mild, I tried to take things in my stride most of the times although there still have been the odd incidents and one specifically where I had to go to the local police station. My friends and colleagues at work do find me a bit too aggressive and impulsive at times. My dad passed on in the year 2005 and all the litigation and court cases were wound up after that. I landed a good job by 2008 october and we moved back into the house where we used to live. I still did not maintain or allowed anyone else in the family to maintain any relations with the father's side of the family till date.

Education: I did my schooling from Choithram School till 10th Grade (1994) and then at IPS till 12th(1999), then I did BCA(2002), finally did MCA from IGNOU (2007) and then did a certification in .NET from Microsoft.

Occupation: I am currently working with a MNC (WM Logistics) in XXXXXXX my hometown, as a Principal Software Engineer since November 2011, before that I worked with another MNC (IIT Inc.) in XXXXXXX itself since October 2008, before this I had 3 years of experience in another firm as XXXXXXX Business One Add on Developer and worked as a freelancer on rentacoder.com in 2002 till 2004

Family: My family is of 5 members which includes me, my mom, two younger brothers who are twins and the wife of the younger of the twins. the older of the twins lives and works in London, UK, the other twin and his wife live here with me and mom. the twin who lives here also works at the same company but in a different department. Me, my brothers and Mom are all very close to each other, My brothers and Mom are especially worried about me, my mom is worried about my marriage and finding a wife who is understanding and caring and is able to adjust with me and take care of me. The younger twin told me that if you dont married I am not gonna have kids and the other twin is not going to get married even. So overall I am turning 35 next month and I weigh 115Kg I wouldn't bet on me.

Prescriptions: I have uploaded a pdf file with the prescriptions given to me till date, also one more thing I would like add is that the current prescription that I am on is affecting my sexual function, since I started taking the medication I have not been able to masturbate.

Brief Answer:
it is likely bipolar disorder, let me know current medicines

Detailed Answer:
hello,

thanks for detail information. I read it carefully.

From not sitting in exam of 12th to the successful job, you have been undergone very stressful situations in life.
Considering your history, the way you narrated your problem clearly and weight gain from medicines,I guess you have bipolar disorder.Again to confirm diagnosis many cross questions and history from relatives are necessary.

You should marry or not that would be a tough decision. This decision must be taken after discussing your problem with your psychiatrist and family. You all should reach to some conclusion.
The girl whom you are going to marry in future must be informed about your problem before fixing the marriage. If you are stable on medicines, I think you should go for marriage. But the best conclusion can be made by discussing with your treating psychiatrist.

In your attachment all prescriptions are old. Please let me know current medications. I can throw some light on medicines responsible for weight gain among them and can tell alternatives of the same are available or not.
Weight can also be reduced by exercise, diet changing and lifestyle changing.

Hope you have got satisfactory answer, do not hesitate to ask if more queries.
