What Does My Ultrasound Scan Report Indicate?

Question: Hi,

I have done a medical abortion with the help of misoprostol (200mg), after abortion the ultrasound report saying "UTERUS: is normal in size (4.2x5.7x8.8cm), shape with smooth serosal outline and well seen endometrial echo, which is 11.4mm thick as on today, appears split due to presence of minimal fluid in the endometrial cavity.", then doctor again given misoprostol (200mg) for 4 days and after repeat the ultrasound the report was says "UTERUS: is empty, anteverted, normal in size (3.5x4.2x7.4cm), shape with smooth serosal outline and well seen endometrial echo, which is 12.6mm thick as on today."

So in the 2nd ultrasound endometrial is increasing a bit, and doctor recommended to go for D&C. We are scary and avoid to this procedure and would need help if some other option are there to get the normal size of endometrial. Please suggest.

After a medical abortion,. there can be suppression of the endometrial growth or an irregular growth of the endometrium. Since there are no retained products in the uterus and if you are not having a heavy bleeding this can be taken care of with progesterone therapy. The endometrial lining will shed off after you will have your periods next time.

Thanks doctor for your kind of advice. Yes on the 2nd attempt of taking misoprostol the bleeding was very less compare to first. As per your suggestion progesterone therapy can resolve the endometrium, does it mean it gets the normal size after it?

Also should i wait to next period and get examine by a ultrasound so it was shed off every period? Also i don't know why i have lot of pain at my lower abdomen area and having problem with loose motion. Has this related to this reason?

My doctor called tomorrow morning for D&C, please suggest what should i do. I have no baby yet and don't want to face any problem in future.

Wait for your next periods.

Hello.
Thanks for writing again.
Progesterone therapy is needed if you are having any active bleeding right now. Otherwise you can wait for the next period and this endometrium will shed off on its own. There is no need to expose yourself to the risk of complications after D&C since you have no baby till now.
The pain in lower abdomen usually subsides after 2 weeks of abortion. The loose motions can be due to the effect of misoprostol. You can take tablet Lomotil to control the loose motions.

Thanks for your clarification on my doubts.

I was reading one of an article has similar issue -

My doctor says endometrial can be < 7 will be normal, but in my case it is 12.6mm which is quite higher. So when i was read above article it was suggest "If the thickening still persists, unfortunately a D and C will be needed as it may get infected and hamper future fertility", so in my case if endometrial keep increasing it may cause a problem and might be make problem on future pregnancy or become cancer. Has endometrial related to residual live tissue or part of womb?

If there is a solution available, I am confused why my doctor recommending D&C instead of waiting next period. I have done a Urine pregnancy test and it was negative, and as per latest ultrasound (second-ultrasound.JPG) everything seems okay except the endometrial. If i wait for next period the endometrial can't be getting more larger and make worst case?

Please suggest.

Chances of infection are rare.

I have seen the mentioned page. It is rare to have an infection due to overgrown endometrium if a medical abortion is done with the help of pills. Infection is more common with D&C itself.
It will not turn into a cancerous growth and even if in rarest cases it does then a D&C cannot stop the extra proliferation.
I personally do feel that you should wait for your next periods. Do consult a gynecologist in case there are signs of infection like pain, high fever or abnormal discharge from vagina.

Hi Doctor,

As per your instruction i could wait for next period, however i was suddenly bleeding started from 29th Dec to till today. I was not sure it's my menses started which was generally came on 5-6 date on every month, although bleeding was not heavy and its quite normal which now stopped today. I am curious to know what will be my new date, does it normally follow the old date or may follow new date after pills taken (that is 22th Dec) or 29th Dec?

Also i found an old ultra-sound report(see attached old-ultrasound.JPG), which was done in Sep month when i was normal, so based on report on that time i have Endometrium 9.6mm, and now 12.6mm which not looks too much increased. I am not sure what is normal endometrium a woman will have.

When i visited to my doctor he recommended to take Uniceph 500mg pills to avoid any infection (please see attached doctor-prescription.JPG), is this okay to take this?

Irregular bleeding can happen due to the effect of abortion pills.

Detailed Answer:
I have seen the attached report and the prescription. The thickness of endometrium varies with the hormonal changes during the cycle. A thickness of upto 15 mm can be considered normal just before the periods.
The bleeding that you had now is due to the side effect of the abortion pills that you took. Such irregular bleeding can persist for 3-4 weeks. Your next period will be around 22 nd Jan- according to the date when you took the abortion pills.
