Bilirubin will take time to come down

Hello! Thanks for putting your query in HCM. I am a Gastroenterologist.

I can understand your concerns

Jaundice usually takes time to lower down than SGOT and SGPt.

Give him high protein and high caloric diet. This is the biggest mistake which most of the patient do and they restrict the diet. So give him high caloric and high protein diet. Also give him fresh fruits and vegetables.

Also add Tab Udiliv 300mg bd

Keep watch for sign of liver failure, like sleep disturbance, tremors in hands, loss of memory.

Keep in close touch with your doctor with regular follow up

I hope he has undergone tests for hepatitis like Hepatitis B, Hepatitis C, Hepatitis A and Hepatitis E

I hope I have answered your query and this will help you . Wish you a good health

Hepatitis E is positive
Till what levels bilirubin can go?..doctor is saying it would come down slowly..can it increase more? What physical signs should we keep a watch at?
Does increase in bilirubin indicate liver failure when other factors are reducing?

Brief Answer:
Bilirubin should start decreasing now

Detailed Answer:
Welcome back

Bilirubin should start decreasing now

I have already mentioned sleep disturbances, memory loss, tremors in hand, sweet smell from mouth

When other factors are reducing, means the things will improve

Liver failure is not seen in Acute Hepatitis unless there is any already existing disease in liver. It is 100% curable at least in males. So keep cool, since SGOT and SGPT have started improving, the other things will also improve but keep in regular follow up with your doctor

One more thing I want to know is exact report of ultrasound abdomen

Wish him a good health

Brief Answer:
consult hepatologist or gastroenterologist

Detailed Answer:
Welcome back.

I have gone through the report of sonography. It suggest that he having underlying chronic liver disease that is cirrhosis.

And now there is superadded Hepatitis E infection leading to liver failure.

I will suggest you to keep him in close follow up his doctor. One more thing consult gastroenterologist or hepatologist if you are visiting any physician only as this is not a good disease and overall has poor prognosis.

One more thing it is not clear why he developed cirrhosis? If he is diabetic or he takes alcohol and what were the results of hepatitis B and C tests

Wish him a good health

Brief Answer:

Close follow-up with your doctor is recommended.

Detailed Answer:
Welcome back.

Good to know that you are consulting gastroenterologist because he can handle him well.
So both diabetes and alcohol are the cause of his chronic liver disease
Keep his sugar controlled

Ask him to keep total abstinent from alcohol.

Take care of him as already discusse.

Wish him a good health
