What Does My Lab Test Report Indicate?

default
Posted on Tue, 10 Nov 2015
Question: I am 34 week pregnant now, today I had scan. But in scan report its showing me only 31 week. Fluid is also low and baby weight is only 1.75 KG

AFI: 9 to 10 cms
BPD: 7.2 cm
HC: 28.2 cm
FL 6.4 cm
AC: 26.6 cm

I have also seen Umbilical vein wavefor: Non-pulsatile. Not sure if its reason to worry.
I am worried now, can you please provide your input. What should be my next action of plan ? and is it really worrying result ?

Brief Answer:
slight variability is normal

Detailed Answer:
Hello dear, welcome to healthcare magic. I am glad to meet you again.
I can understand your concern.
I have seen your ultrasound report.
In my opinion normally the umbilical vein doesn't show any pulsations and presence of pulsations is considered as abnormal.
The amniotic fluid of about 9-10 is normal. It is not considered as low liquor.Take adequate fluids upto 3 litres per day as dehydration also leads to less liquor.
And check for fetal movements daily. They should be atleast 10 in a 12 hours span.Usually when there is less liquor there will be decreased movements.
Coming to the parameters like HC,AC,FL and BPD they might be considered as normal because a variability of 7-10 days might be normal in relation to gestational age calculation by dates and ultrasound.
AC is most important parameter in relation to fetal growth and it is good.
Ultrasound might need to be repeated after 2-3 weeks to see for the growth. So discuss regarding that with your doctor.
The fetal weight is appropriate for your gestational age and maximum growth occurs in last months.
So nothing to worry.Check for the movements daily and consider repeating the ultrasound.
As there are no risk factors like hypertension, anaemia etc there is no chance of any growth restriction of fetus.
So relax.
Hope this satisfies your query.
Feel free to discuss further.
I am always there to answer.

Thank you for your detailed answer
I have few more question
1) When you say fetal movement that means feeling of baby moving inside my tummy, right ?
2) in report BPD is showing 29 weeks while I am 34 week, its almost 5 week different, any idea why is it like that and any action/medicine/diet to take to get it normal
3) AC is also showing 30 weeks while I am 34 week.
4) Generally for 34 week, weight is approx. 2.2 to 2.5 that I see while doing some study on google :) and also doctor who did ultrasound told the same. But in ultrasound weight was only 1.75 :(. Do you think it will increase in coming weeks ? or anything I need to do it here ?
5) One point I forgot to mention "fetus seen in breech presentation". Anything to worry on this ?

Thank you again for your help and support

Brief Answer:
High protein diet is advised

Detailed Answer:
Hello dear,I got your query.I understand your concern.
In my opinion following are the answers for your questions:-
1)Yes the fetal movements are the movements felt inside the tummy.
2)The variation of 5 weeks of BPD might be familial too. So it cannot be taken as abnormal.
3)AC appears to be slightly less but a single measurement cannot be taken into consideration.
It needs to be monitored by serial ultrasounds once every 2-3 weeks.
Usually low AC is seen in cases of intrauterine growth retardation(IUGR).
IUGR is usually associated with abnormal Doppler findings. But the Doppler is normal and the liquor is adequate and there are no risk factors like high BP,anaemia so the chances of IUGR are rare.
4)The fetal weight is according to the ultrasonic parameters.Dont worry. There will be increase in weight in the last weeks.
5)The breech presentation is routinely found at this gestational age.By term it will turn to cephalic presentation.
To my best knowledge the difference in parameters appears to be due to familial and racial factors.
As the figures might vary between different countries.
Anyways abnormalities if there can be found in further ultrasounds.
Take high protein diet like egg white daily. And consider doing HB to rule out anaemia.
Avoid stress as it might affect your fetus.
Hope this satisfies your query.
Feel free to discuss further.

Thank you Dr Srilatha for detailed reply.
Today we visited doctor and she mentioned main worry is that "umbilical artery decreased". Looks like blood flow is less from Umbilical cord to baby.
And same as you metioned to have more protein food.

Brief Answer:
It needs fetal monitoring

Detailed Answer:
Hello dear,I got your query.I understand your concern.
In my opinion if Doppler is normal then there is nothing to worry.
But as it showed decreased umbilical flow it
also doesn't need any urgent intervention but it needs closer fetal surveillance to determine whether or not there is fetal deterioration further.
Closer surveillance like ultrasound and doppler once in 2 weeks might be needed.
It might revert to normal in some cases.
Continue high protein diet and take rest.
So relax.
Hope this satisfies your query.
Feel free to discuss further.
Best regards...Dr.Srilatha
