What Does My Blood Test Report Indicate?

Question: Dear Doctor,

I am a Male of XXXXXXX Nationality born on 16/09/1981 with 166 cm height and 89 KGs Weight, Yesterday i have got my casual blood work done and reports are hereby attached for your reference.



Peculiarities in Report:

1) High Serum Calcium (10.03 mg/dL)

2) Low ESR (1 mm/Hr)

3) Low Alkaline Phosphate (36.80 U/L)

4) Low Vitamin D (19.90 ng/mL)

5) High Total Cholesterol (231.99 mg/dL)

6) Non-Obstructive Kidney Stone of 2mm

7) Blood Pressure 146/82



My Present Condition:

1) Breathlessness after walking for some time which usually releaves after taking a puff of Asthalin Inhaler as i am having Paternal Asthma which is elevated only when in contact with Cold Climate or Dust.

2) Left Leg Joint Pain occasionally.

3) Sometimes Stiff Neck and feeling of hotness on the neck.

4) Sometimes Stabbing Pain above my buttock at the end of back bone.

5) Generalized feeling of fatigue usually in the evening after office hours from 4 PM to 8 PM.



My Primary Question:

Do I have Hyperthyroidism related Cancer or any other Complications?



My Secondary Questions:

1) Why my Calcium is on a higher end and Vitamin D on a Lower end together?

1) Do I have cancer/tumour related to Thyroid Gland?

2) Any Tests are required to diagnose further?

3) If no tests are required then what should be the duration of Wait-and-watch?

4) Is their any need to personaly visit an Endocrinologist?

5) I am taking Vitabiotics Cardioace (Vitamin Supplement), One Capsule per day since one week which is having 20 Micro Gram Vitamin D. Still I am deficient of Vit D, Why?

6) What is your Overall Recommendation on the conditions mentioned above and what supplements you will suggest to relieve from the situation.

Hi,

I have gone through your medical history carefully and understood your concerns.

First, we cannot determine whether the cancer is present unless having further diagnostic tests.

1. Your calcium levels are only slightly elevated only few points above the upper range. That is not worrisome.
On the other hand, your vit.D levels is moderately reduced.

It is true that when persisting high calcium levels associated with low vit.D need further investigations to rule out parathyroid problems. PTH (parathyroid hormone) test and thyroid ultrasound (where parathyroid gland is checked too) is advised to rule it out.

But in your case, I do not think it is an emergency for the moment.

1. Parathyroid problems should be ruled out first as advised earlier.

2. With regards to tests: PTH test and thyroid/parathyroid gland ultrasound advised.

3. As I did not notice to have a chronic kidney disease, I advise to consult with endocrinologist and discuss about parathyroid problems.

However, that is not an emergency, can wait for a couple of months until seeing endocrinologist.

4. Endocrinologist advised to consult in a further step.

5. It will require at least 2 to 3 weeks to see differences on vit.D level prior to determine its effect.
Repeat testing after 1 month.

6. You are having slightly elevated cholesterol levels. Changes in your diet and lifestyle regimen advised for the first 3 months.

- avoid foods high in calories and sugar
- limit salt intake
- eat more fresh fruits and vegetables and foods high in fiber
- red yeast rice, guggul and garlic supplements can help reduce cholesterol levels
- physical activity (moderate to high intensity) can also improve your cholesterol numbers. However, you should avoid those activities that would aggravate your asthma problems.

I have also noted that your blood pressure was also slightly elevated. I advise to monitor your bp, record the results and pass to your doctor to decide for the best treatment.

For the moment, limit salt intake can oily foods can help improve cholesterol numbers too.

I have done my lipid Profile recently and found out the following.



Total Cholesterol: 231.99

Triglycerides: 111.39

HDL Cholesterol: 77.28

LDL Cholesterol: 141.11

Vitamin D: 19.90



My GP has told me to take "LIPITOR 20mg ATORVASTATIN" daily once for 3 Months and Ergocalciferol once weekly.



My Question:



(1) Is it necessary to take this medicine or life style change is enough.

(2) Do this medicine will be habitual and i will end up in using the medicine for lifelong.

(3) I am using Vitabiotics Cardioace original, one capsule daily since last one week. Can i continue the same with Lipitor or either one will be fine.

Hi XXXX,

Thank you for following up.

1. Considering your actual lipid profile and abdominal ultrasound (where no liver infiltration or no fatty liver) was found; if I was your caring doctor would not advise to start Lipitor for the moment.

The standard therapy advised when a patient comes to my clinic with a recent lipid profile with altered cholesterol levels, includes lifestyle and diet changes for 3 months.

I always ask my patients to repeat lipid profile testing after 3 months of applying such changes to see if any improvement. Only then, we can decide on taking lipitor.

With regards to ergocalciferol, I advise to continue taking it as prescribed to see if vit.D will be raised.

2. The very first thing is to determine whether there is chronic disease causing such alterations. Usually, they are temporary, and won't need for habitual taking of such medications either anti-lipemiants or ergocalciferol.

3. Vitabiotics Cardioace original contains mainly the extract of garlic and flaxseed oil together with other essential multivitamins and minerals. You can take it safely even if you decide to take Lipitor.
