What Does My Abdominal Ultrasound Scan Test Report Indicate?

Question: With refernce to earlier question . As the pain didn't decreased we moved to another doctor . Abdomen ultrasound was normal . Mild decrease in hydrocele and inflammation epididymis . He ruled out the possibility of having filariasis . And gave treatment for epididymitis and tied bandage to the affected area and advise to apply hot water bag .
Pain is much reduced . And he is going without painkillers . But it has not ckmpletely been cured . What will b the next course of treatment . We hail from XXXXXXX . Which hospital / doctor would be best in XXXXXXX for better treatment and diagnosis .

I have read the case in detail and it seems you have been striving hard for the correct treatment. However, most of the treatment has been on the right lines. What I feel is that the Ultrasound report of the scrotum showed microfilarial dance which is seen not very commonly and if it is seen it is usually confirmed that the patient has filarial worms. Therefore, the treatment of DEC would be right in that context and no Doctor should refute that. The treatment protocol for DEC is usually for 12 to 14 days continously and we usually prescribe two tablets thrice a day, i.e. 100mg.
The persisting pain could be due to a secondary infection. Is he having burning in urine or blood in urine? If not then that is unlikely. However, I would still advice a test of Urine Culture which would tell whether there is infection in the Urine or not. If not, then nothing to worry about. If there is, then the test will also suggest what antibiotics to give. Those antibiotics can be started for a period of two weeks and then stopped. This should give him good relief.
Pain killer medicines can be taken as a n when required under the cover of anti acid medicines like Pantoprazole or Omeprazole.
Advice him to wear tight underwears so that the scrotal sac does not hang which can increase the pain. The treatment protocol is fine and you need any good Urologist to be consulted by.

Is filarial infection completely curable ? Will DEC completely cure or edema will still continue ? What precautions need to be taken ?

DEC is effective. Edema will take some time to resolve.

Hello,
Filaria is quite treatable.
The complications of Elephentiasis, etc are quite rare and happen in ignored and untreated patients. DEC is the drug used for 12 to 14 days and it is supposed to effect complete killing of the worms.
Edema can continue for 2 to 4 months. It takes a slow time to resolve but pain should not be there. Pain, burning would be suggestive of super-added infection which would require culture specific antibiotics.
Precautions in the form of healthy balanced diet with a lot pf proteins, tight supporting undergarments and good personal hygiene.
