Question: Hi I m so happy that your write i was just thingking to ask you another question , i am fine so far, I has and MRI done a few days ago, after the TIA diagnosis, I am feeling a lot better, but the MRI result show that their is a scattered foci increased flair and T2 signal intensity in the frontal subcortical white matter of both hemispheries. Differential include small vessel changes, vasculitis, migrane encephalopathy and postinflamatory residuals. I know this is a neuro matter but i just want to have an idea is this something to worry about or this is from aging. I also went to my PCP and she added omeprazole 40 mg daily to my regimen it seems like I had heartburn. I does not feel that heat and chest discomfort any more. I also have at times a twitching right above my left eye at times. My eyes also tend to get red is this maybe side effect from my Metroplol/HCTZ regimen what can I do. thanks so much. Oh I suffered from a lot of headaches ( at times). Thanks. Oh my Hematocrit increase to 12 as per labs i did last week.

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Your MRI scan brain shows that there are scattered areas in the white matter located in the front portion of the brain on both sides and this is called the frontal lobe. The increased FLAIR and T2 is a characteristic of the signal changes seen in these areas of the brain. The findings though not an emergency at the moment, however might be due to the following conditions

(i) Microvascular changes are due to long term decreased blood flow through the small vessels in the brain. This can happen in people with hypertension. diabetes or also as a part of advancing age.

(ii) It can also be due to inflammation of the small vessels in the brain that is causing reduce blood flow.

(iii) Migraine headaches might also appear this way on MRI scan brain.

(iv) Old infections that happened months to years back can also show these findings.

(v) Early aging changes might also cause these developments and this can happen especially in hypertension.

As you have rightly pointed out, a complete neurological clinical examination is required to confirm any neurological condition and to start the required treatment. Your headaches might be associated with the above findings.

Taking omeprazole might be helping you in reducing the formation of gastric acid and therefore no more heart burn.

Eye and muscle twitching is rarely seen in those taking metoprolol and hydrochlorthiazide and needs to be discussed with your doctor. They might offer a change in medicine.

Your hematocrit value is probably the hemoglobin that you have mentioned as 12. Hematocrit by definition refers to the ratio of the volume of red cells to the volume of whole blood and is 37% to 48% for women. Please reconfirm if you intended to write about your hemoglobin which is normal at 12 or hematocrit.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Hi XXXX,
Welcome and thanks for writing back with an update.

Your hemoglobin is 12 and that is lower limit of normal. The normal range for females is 12.1 to 15.1 g/dL. You can include healthy food in your diet to elevate the hemoglobin to a mid normal value.
