Brief Answer:
Could be something else!

Detailed Answer:
Hi and thank you so much for this query.

I am so sorry to hear about this about this bad breath that you have reported. With thorough brushing and persistence of this bad breath, the chances are high that the cause of this bad breath is further down. Two possibilities are the lungs and the esophagus. Esophagus may at times develop a small pocket that collects food and allow it to rot. This causes a bad mouth odor in this case. Another scenario is a persistent infection in the lungs that may cause this.

Seen the fact that you have tried several options already, I will like you to follow up with two specialists. If you have any difficulties or discomfort swallowing or in the neck region, then I recommend you start with a gastroenterologist. He would most likely look down to make sure there are no pockets that may serve as reservoirs for food collection.

If You have any breathing difficulties or chest pain, then I recommend you start with a pulmonologist for exploration. He would most likely look for a silent pocket of infection in the respiratory passages.

If the first doctor fails to identify the cause, then please followup with the second.

I am confident this approach would most likely provide answers to our questions. Let me know how you go about this and whether it is a plan you would be following up with.

I hope this helps. I wish you well. Thanks for using our services and do feel free to ask for more information and clarifications if need be.
