Question: I have been off contraceptive pill for 2 months now after 20 yrs, and been taking folic acid 5mg. Had tender breasts for the past 2 period cycles and have my period currently (day 4). Noticed that I have a hardened cyst like lump that has formed beside my left nipple and is very tender. I also think I knocked my nipple on the weekend, and also in my sleep. What could this be caused from?

Thanks for writing to us with your health concern.
After going off the contraceptive pill, it can take upto 2 - 3 months for the hormonal effects to wear off, particularly as you had been on it for 20 years, a pretty long time.
The breast tenderness could be due to that.
THe hardened lump could be caused by the nipple injury, leading to accumulation of fluid / blood inside the lump.
A proper physical examination is a must to confirm the diagnosis.
Is the lump noticeable - if so you can upload pictures.
It is vital to consult a doctor for proper examination .
Generally, tight packing and cold compresses would resolve the lesion if it was due to an injury.
How big is it ?

The lump is not noticable, only to touch. The one thing i do notice is the nipple looks off centre like its bren pushed out of centre. There is no discharge or redness, just very tender

The surface lump looks more noticable in the photo. See it is beside the nipple leading in to it. No discharge but nipple is larger than the other one. The rest of my breast doesnt hurt at all, just this area. I am more and more positive i have knocked this. I read online that folic acid supps cam also cause breast tenderness in my agegroup. Is 5mg too high a dosage to take? My husband and i are also trying for a baby, could this lump perhaps be a pregnancy sign?

You mentioned you are on day 4 of your period, so how do you consider pregnancy as a possibility ?
Secondly, it is a very small lump, most probably due to trauma.
Just wear tight breast support and apply local ice packs.
Folic acid supplements can cause breast tenderness but 5 mg per day is an apt dosage, it is not too high, you can definitely continue taking it.
