What Do These Following Lab Reports Indicate?

Question: My wife is underweight and as per the recent tests done there are few parameters which needs attention, could you please help me to provide the detailed analysis along with correct line of treatment as she is approaching around 40yrs. Is she required to take iron/folic acid/vitamin D/Calcium/proteins regularly?
Please also suggest in case of any other tests which are required.
Waiting for point wise answers and suggestion on food habits.

Thank you for asking HCM.

I have gone through your query. From the report calcium is low. There is sign of bacterial infection in body (increased neutrophils). And there is hypochromic anemia which denotes iron deficiency.
It is better to do blood tests for vitamin D and vitamin B12 levels in blood. ESR , C reactive protein , Urine routine test and stool routine tests. If ESR is high TB PCR for tuberculosis should be done also.

I would suggest worm eradication by drugs like albendazole 400mg as single dose. For anemia iron supplements will be required. I would suggest brands like dexorange capsule or syrup 15ml twice daily for this.
For low calcium i would suggest 500mg calcium plus 400 IU vitamin D twice daily.

If vitamin D and vitamin B12 are low supplements should be taken for those also.

From stool routine we can find out whether there is any malabsorption syndrome. In such case it should be treated.

She can take protein supplements like B protein powder daily.

SGOT SGPT and serum protein.

As per current report if you were my patient i would suggest

1. Iron supplements - like Dexorange capsules twice daily or dexorange syrup 15mltwice daily.
2. For worm eradication- Albendazole 400mg as a single dose at night.
3. For low calcium- Calcium carbonate 500mg plus vitamin D 400 IU twice daily.
4. Protein powder like - B protein powder to take with milk for weight gaining. This can be taken 30gm for 1 serving once or twice daily.

It is mandatory to do the above tests to find out any other problems causing this. You one which is not that mandatory are vitamin B12. This you can take supplements which won't cause any problem. ESR and C reactive protein for finding out any infectious or inflammatory process running in the body as there is increased neutrophils in blood report. Vitamin D levels to find out if low vitamin D is the cause of low calcium level. Urine routine to rule out urine infection. And stool routine to rule out malabsorption syndrome which can be a cause of weight loss.

Albendazole can be taken once yearly. But in case of worm symptoms it can be taken once in 6 months also.
If she is non vegetarian then she can take red meats like mutton , eggs , chicken , fish etc.
If vegetarian taking nuts like almonds , peanuts ,cashews or nut butters, milk and milk products like cheese , paneer etc.
Citrus fruits like orange or lemon should be consumed also.
Enough fluids should e taken also.

Reason for advising albendazole is anemia.

One of the reason for anemia and weight loss is worm infestation. So it is a routine to do worm eradication. That is why i advised deworming with albendazole.

As a vegetarian she can take milk and milk products , various nuts and nut butters, soya beans, fruits like avocado etc for weight gaining.

Pl suggest in case of any side effects for taking Dexorange syrup and which brand is good for Dexorange syrup, as she is already taking protinex in milk once daily in the morning with milk so talking calcium carbonate 500 mg tablets are required?

Also suggest the duration/time for taking Dexorange syrup so that tests can be repeated again.

Dexorange is a brand of iron syrup and softgel capsules. Usual side effects are gastro intestinal upsets. If she can't take syrup because of the taste then can go with softgel capsules. There are also brands which has less Gastric side effects like Niferex 150. This you can take twice daily.

Calcium and vitamin D supplements are needed as milk alone is not sufficient to cover the deficiency. Calcium and vitamin D combination also twice daily.
This both should be taken for 3months. Follow up blood tests should be done after 1 month.
She can continue proteinex which is a good protein supplement.
