Posted on Mon, 22 Jun 2015
Question: Hi, on the 31/03/15 my TSH was 8.6 and my Free T4 was 12.
My Doctor then took blood on the 21/04/15 for the Anti Thyroid Peroxidase test, this came back as a reading of 679.0 U/ml the Normal Range was given as (0-50). This was done in the U.K

Can you advise what these readings indicate and how severe ?

Thank you

Detailed Answer:
Hello.

Welcome to Healthcare Magic.

I read carefully your query.

Considering your test results you have Hypothyroidism, which is an underactive thyroid gland that produces less hormones.
The level of TSH is not very high ,which means that you have a mild grade of hypothyroidism and the dose of Levothyroxine 25 mg is very low dose.

The presence of TPO antibodies (Anti Thyroid Peroxidase ) in your blood suggests that the cause of thyroid disease is an autoimmune disorder, such as Hashimoto's disease .In this case your immune system doesn't recognize an enzyme called Thyroid peroxidase as it's own and produces antibodies to fight it,which are called autoantibodies.
TPO is an enzyme found in the thyroid gland, that plays an important role in the production of thyroid hormones.

The destruction of this enzyme cause the reduction of thyroid hormone production. This causes hypothyroidism.

You don't need to worry. This condition can be totally managed by taking a low dose of thyroid hormone (Levothyroxine).

Hope it helps.
Let me know if you have any question.

Kind regards,

Dr Murati,

Does this condition get worse over time (and therefore would require an unavoidable increase in Levothyroxine or can it rectify itself, say with changing one's diet etc)

Kind Regards

Detailed Answer:
Hello.

Usually the hypothyroidism due to Hashimoto's disease( autoimmune disease) is permanent. It doesn't get better with time and is not influenced by diet.
But is doesn't get worse either .After the initial inflammation due to autoantibodies production ,the inflammation settles down and the thyroid function is reduced but stable.
You need to test the thyroid hormones level in regular intervals and the dose of Levothyroxine will depend on these results.

Hope helps.
Take care.
