Question: My boyfriend and I only have sex about one a month because we both travel a lot. However, about 7-9 days after having sex, his testicles swell and he gets a fever for about 24 hours. What is causing this and why the delay in symptoms?

I would say there may have been a bacterial infection in the form of a UTI and its complication is the swelling of the testis which is what we call orchitis.

The incubation for an infection is 3-14 days depending on the organism involved and its virulence(activity). He may need an oral antibiotic and anti-inflammatory to relieve the swelling and eradicate the offending bacteria.

It is generally said to urinate before and after sexual contact to flush away any bacteria that may cause harm. Drinking plenty of water also enhances our immune system against infections at the same time decreases bacterial growth since frequent urination cleans the body by flushing out unwanted organisms.
