What Do My Lab Test Reports Indicate?

Question: Dear Sir,

I am seeking to get some advice from a urologist. This is my friends Kid - Who has just completed his First birthday.
Details are as follows -

1) Immediately after his birthday, we saw some swelling around his penis. We thought it was normal and when we pressed it, it went inside and did not come back for a few days.
2) However after a few days, the problem cropped up again.
3) We saw a doctor in XXXXXXX XXXXXXX who said that he will need to be operated as it is a case of Hernia (He said Hernia in children are different from Hernia in Adults)
4) However we thought of taking second opinion and hence went to Lilavati Hospital XXXXXXX and met with a pediatric Physician, He said it is not hernia and a clear case of Hydrocele.He said no surgery is needed and it will heal itself with time and if not healed in 2 years then we might need to do surgery.
5) By now we had two different opinions and hence went to another doctor (This time a pediatric Surgeon) in Hinduja Hospital, XXXXXXX This doctor again said that it is Hernia and will need to operate.
6) For the last week or so we have not seen any swelling now.

We are now extremely consfused as to wheter It is Hydrocele or Hernia and whether a surgery is needed or not. I am attaching some reports and photographs here for your reference. It will be really helpful if you can have a look and give me your feedback.The child as i said earlier is just 1 year old and we are extremely worried.

It's an inguinal hernia, with fluid around cord.

Detailed Answer:
Hello XXXXXXX and welcome to HCM.
As an Urologist,i can understand your concern.

The ultrasound scan clearly shows,that Reyansh has a R.inguinal hernia.
A similar small swelling is seen on the left side.

A hernia is a swelling,that protrudes out from inside the abdomen towards penis.
In Reyansh's case,the swelling is only for a limited extent of the spermatic cord.
The swelling isn't extending outside,towards the scrotum, all the time.
This is the reason for the confusion.

There's no emergency to get it operated.
You can very well wait for a few months,till the swelling becomes more prominent.

On the left side,there's a very small collection of fluid high up.
So, you can see the changes on that side,by the time he's two years old.
Only if it becomes prominent,it'll need surgery.
A scan will need to be done,before surgery.

A laporoscopic surgery by a pediatric surgeon is advisable.

Dear Sir,

Thank you for your quick response and is really appreciated. I have also attached 3 more images one of which is the report that talks about Hydrocoele. The new images that I attached are 2.jpg, 3.jpg, and 5.jpg. Can you have one final look at even these and give me your final feedback. I am not sure why one report says hydrocele and if that is any different then hernia.

Both reports are basically the same.
The doctor may have communicated differently.
The scan clearly shows the same findings.

So don't worry about terminology and get confused.
In case of any pain,see the doctor urgently.
Otherwise,Reyansh's surgery can be planned when he turns two years.
