Question: My daughter who is 7+years old complained of lower abdominal pain (which goes awaywith the passage of stool or gas) last month with fever (100 degrees) in her urine test she got 5-7 rbc all other findings were normal. she was put on antibiotic and i got her USG done. it was normal. thereafter her KFT and 24 hrs calcium creatinine was also done. both test were also normal except with low calcium levels. she recovered in 2 days and is well since then. However after stopping antibiotic (after 5 days) within a gap of 2 days urine routine was done again. It showed the same RBCs i.e 5-7. after three weeks, it came down to 2-3 rbcs...Shall i investigate her further

Yes you need to investigate thoroughly further since repeat testing even after 3 weeks is showing RBCs even though its decreased.

RBC's in urine indicate hematuria which implies any residual Infection/inflammation process in the kidney.

Its important that you get a repeat scan of the kidneys and complete 24 hour urine testing instead of spot specimen testing.

These two tests taken together should be sent for assessment and further management.

Final conclusion can be made only if these tests are normal.

I received another answer yesterday which says that no further testing is required as she might have an inter current viral illness and has recovered now. However I am going to get her tested further.

By suggesting that i should get a repeated scan of kidney, are you suspecting stone or something?

Let me tell you Mam that I am in a very worried state of mind. Getting a lot of opinions which are different. Not able to think what should I do???

Kindly alleviate my worries. I need to get her treated and cured completely in a best possible way.

Concerning the age of your child,its important that Glomerulonephritis or Major organic Kidney lesion has to be ruled out since Hematuria(RBC in urine) is a significant pointer of these two conditions.A stone can also be a possibility.

A normal renal ultrasound would virtually rule all this out.

A 24 hr urine exam would help to form a corroborative evidence.

Hence it is mandate to get both these tests.

An inter current viral illness rarely produces hematuria.

I appeal you to be calm and not anxious.

Do not get carried away by various opinions.

I am the father of the child and unlike usual, I am more worried than her mother.
and your reply has made me more worried. I don't know how should I control myself.

I will get her further tested no doubt. 24 hrs test for what...calcium, creatinine or protien. I like to know why you said, concerning the age of your child

I hope treatment to both the conditions you mentioned is available.

Please help me madam. I have become a patient myself.
