Brief Answer:
Antibiotic and orthopedician visit

Detailed Answer:
Hello sir, thanks for trusting health care magic.
The symptoms of vomiting, fever and dysentry are probably related to some stomach infection due to infected food which is quite common in rainy days so you need to take antibiotic metronodazole thrice a day for 5 days and take lots of curd and yakult so that your stomach gets good bacteria to fight the infection.
Avoid outside food for a while from now.
MRI is clearly suggestive of tenosynovitis of spine in lumbar area and thus causing backache.
For this you need to visit an orthopedician because exact physiotherapy can only be advised in person and after thorough examination of body movements.
all the very best.
