Question: Hi Dr
I have a very tight anus. I've been checked for fissures and it was negative. I've used suppositories with no relief. The pressure in my anus is extreme and is 24/7. It makes getting an erection impossible. It constantly feels like I have to poop but I don't. I try to push the muscle to relax and it stays with the same pressure.

The causes of such an occurrence in a young man of 20 can occur due to the following reasons:
severe procto-colitis
Descending perineal syndrome
IBS- irritable bowel syndrome
Prostatitis
Rectal polyp
This can be checked and confirmed by the following:
- Per-rectal digital examination.
- Proctoscopy to start with
- Colonoscopy and biopsy.
- Ultrasonography of the whole abdomen, particularly focusing on prostate and the whole genito-urinary system too.
- Clinical evaluation by a General Surgeon and/or a Gastroenterologist.
- Tests of stool for consecutive days.

This way it would be easy to make a proper diagnosis and plan a further management.

Can you please provide more details as to what is prostatitis, descending perineal syndrome, and procto-colitis is and what symptoms are experienced with each of them? Also how can any of them affect erections?

Details:
Prostatitis: Is inflammation of prostate gland, which in young man of your age is usually due to infection. Per-rectal examination, Ultrasound and Urine examination can help to get the proper diagnosis and treatment. One get pain deep within the perineum, painful urination, difficulty in urination. Many a times the symptoms can be vague.

Descending perineal syndrome: one gets the continuous discomfort and feeling like to pass motion and strains a lot but does not pass any motion.

Procto-colitis: is inflammation of the colon, rectum. The symptoms are the same as you have explained.One needs colonoscopy and biopsy to know the exact diagnosis.
All can have similar symptoms with mild variations as discussed above.
All the diseases are near the prostate and the apparatus that is responsible for normal erection.
Secondly erection is a process which has the psychology as the most important constituent. Even a small pain may not allow the erection. Trivial things like not feeling well will not allow the erection.
I would advise you the following:
Get a proper diagnosis by consulting the Specialists in the field, get detailed investigations to know the perfect diagnosis so that you can be treated properly.
