Question: thank you, I am an 82 year old guy, still working 7 days a week, in major real estate development and construction in 6 states, but love my work so much that I can!t stop, i however I am in a position that I am basically in a 0 stress situation, 14 years ago I became a serious type 2 diabetic, so serious I jumped over the metformin stage and it was necessary to take 70 units of insulin ( HUMILIN) morning and night, my a1c at one time ran as high as 9, 2 years later as expected here comes stage 2 hypertension, at one time 5 medications 3 times per day, I did some research and started balancing all of my hormones, in 2 years to the amazement of my pcp, I eliminated my diabetes, haven!t had an a1c over 4.8 for over 1 year, I have balanced all of my hormones as well as my thyroid which when I started was 67 % deficient, my blood pressure meds are down to 3 morning and night, still a bit high but still improving, about 6 months ago I started having swelling a little bit in my ankles, but much more in the tops of my feet just a little forward of the arch in my feet, the swelling is not painful at all but is quite puffy, would you think that may be due to some blood flow restriction ? my pcp is like most, unless its of an emergency nature, he has at a XXXXXXX about 15 minutes on a regular visit, do you think I should have any specific test to determine the cause , or what have you experienced in this area, please help me with my country boy PHD, I!m XXXXXXX XXXX

I am really amazed to know (alike your PCP) that diabetes is under control and number of medications for hypertension is reduced over a period of time.

First and foremost cause for swelling of feet for 82 year young men on the hypertension medication is cardiac condition followed by hypertension medication. To rule out the cardiac condition it is extremely important to have echocardiography which will determine overall functioning of the heart. In addition it is important to have the thyroid status as deficient thyroid is another common condition which causes leg swelling.

In case both echocardiography and thyroid status come normal than the most possible cause is hypertension medication. Having a temporary withdrawal and than reintroduction of each medication will determine the offending agent.

the question I asked earlier to your sexoligist related to ejaculation of sperm, at 80, the first time ever to have my testosterone tested my total was 242, my free was 1.99, I had a compounding pharmacy provide me with a 6 months supply of cream which I applied to my inner wrist x 1 for 10 weeks decided to have private blood test done to see if I was making progress, to my XXXXXXX my total was 1800 and my free was 30, I was trying to get my total to 700/900 and my free to 25/30, I immediately suspended treatment but am unable to excrete sperm during intercourse, there is no pain or discomfort, but just basically nothing, I wonder why? its possible that 82 year old s are not to do it anyway, please provide me with any wisdom you have that will help me get over it............

Thanks for reverting back to me.

First of all there is nothing related to age which can block orgasm/ejaculation in a healthy men including high testosterone. The most common reason for not having ejaculation is enlargement of prostate which is common with advancement of age. I have no information about your prostate but if it is enlarged it could be the reason for no ejaculation.


The second common cause for delayed or no ejaculation is medications used for prostate, hypertension and depression.

So it will be appropriate if you can list the name of medications you are receiving along with details of any other medical condition.

thanks for responding so quickly, in the past I have had stage 2 hypertension for about 3 years, it is now under control, my medication are, clonidine 0.3 mg, 2.5 minoxidil, 5 mg amlodipine, 20 mg lisinipril all 2 times per day, morning & night my pressur averages over a 5 day period, nights 140 average, mornings sometimes high as 160 average, we are working on getting the systolic to average below 150, my diastolic is always below 60 and my heart rate is almost never over 60 and often is as low as 48, I feel better when my systolic is between 135 & 150, seem to have more energy. I still work 7 days a week, all administrative, I have started recently to riding a stationary bike in my office 30 minutes 3 times per week, I always feel great, played professional baseball with the Cincinatti reds for 3 years, won 3 national road racing championships have been a private pilot for 45 years,wore out 4 private planes, for a senior/senior citizen, just trying to conquer the world, thank you for your help and advise, XXXXXXX XXXX

had a XXXXXXX moment there and forgot to ask how to test the size of my prostate and if you know any ways to reduce it thanks again..............

Clonidine is probable cause for no ejaculation

I feel lucky to answer you as you are full of energy and life at a age where most people give up.

Regarding the ejaculation thing I think Clonidine is possible reason for no ejaculation. There is some data in scientific literature which suggests clonidine inhibits ejaculation.

Regarding prostate size it can be determined by simple ultrasonosgraphy. If there are symptoms suggestive of prostate enlargement they can be controlled with medication like Prazosin but there is no technique to reduce the size of prostate other than surgery.
