What Causes Small Painful Sore On Inside Of Bottom Lip?

Question: I woke up with a small painful sore on the inside of my bottom lip. It has a small white center.

I am currently on Carbamazepine XR 800 mg daily for roughly the last 6 weeks for seizure management. My obvious concern is SJS ( Stevens Johns Syndrome). Although I dont have a rash on my body or fever, its odd because I typically never get canker sores.

Not sure whether to ask a neurologist or dermatologist on here.

Hi, Welcome to HealthcareMagic,

I have gone through your question and also the clinical image you uploaded.

Clinical picture uploaded by you is in favour of Aphthous ulcer (Canker sore). It is small, painful ulcers on the inside of the mouth, lips, or throat.

Although Carbamazepine is one of the commonest drug causing Stevens XXXXXXX syndrome. At present clinical picture is not of SJS. SJS begins with Flu like symptom (most commonly). More than 10% of the body surface area should be involved including two different mucosal surface.

Keep a close watch, if any macule or vesicle develops on your body other than oral mucosa, stop taking carbamazepine and report.

At this point no more canker sores have appeared. The only issue i'm having is my eyes are burning a bit and slight headache on and off. I read this is a possible side effect of SJS? No fever or rash.

Hi, welcome again,

Burning on eyes without any oral mucosa and skin lesion is not SJS. SJS itself is a disease and has its symptoms. Side effect are talked of only in relation to medicines and not diseases. You may say the initial symptoms.
If any other mucosal lesion or skin lesion appears stop taking medicine and consult the doctor who prescribed.
