Question: age 63. dx RA white matter disease DMII HTN COGNITIVE IMPAIRMENT mild. short n long term memory loss. RF + ANA+ ESR always hi. vaeious skin rashes. itching

I have gone through your history. You are having quite a few co morbitidities.

What I would like to know is, since how long you have been having skin rashes?

2. What are the locations of it?

3. Did you start any new medication recently?

4. If you could message me the medications for your diabetes and rheumatoid, it would be helpful

5. Could you attach the pictures of skin rashes?

The skin rashes could be due to a medication side effect or the ANA positivity you have. But will have to see the rashes to determine the exact cause. Meanwhile you can start an anti allergic like Allegra 120 mg. It would help the itching

Follow up: Dr. Naval Mendiratta (1 hour later)
No current rash. Appears on face, scalp, sore in nose, blisters under tongue. Muscle aches and extreme fatigue. Depression. Anxiety. Metformin 1500 mg. Mtx 6×5 q wk. Celebrex 400 mg. Was on humira. Dr stopped when mri showed spots in brain advanced for age. Could I have lupus? My rheumy says no. Prednisone is only med that makes me not hurt. Auto immune thru out family. Raynouds too-me. Balance off. Falls. Weakness in legs. Sometimes leak feces.

Good evening

Well lupus needs to have much more symptoms than ANA positivity. There is always a possibility of overlap between Rheumatoid and Lupus and the skin rashes could be a part of it. You will benefit from steroids if it is due to lupus and they will disappear soon enough.

I am not sure if you got ENA profile and complement levels checked. ENA profile gives us a confirmation of lupus as it checks almost 25 antibodies within it. And low complements do give us a clue as well. About the clots in brain, I dont think you are that aged to have it. Kindly check for ENA profile and let me know. If it is due to lupus we will get a confirmation through it and it can explain lot of symptoms.

The next time you get those rashes do send me a picture. Also you can consult your rheumatologist on starting Hydroxychloroquine for ANA +ve and the rashes if they are reluctant on prednisolone. Lastly i will want you to take less of Celebrex as it can affect your kidneys.
