Question: I have severe pain in my muscle or tissue area over my waist line on the right hand side on my back - can you help please or would a scan diagnose what it is - i have it many months and it gets worse towards evening time.

I understand that you have severe muscle pain over the area around the wasit line and back. To help you better I need more clarity so that I dont misdiagnose

1. Is it only the muscle/ skin which is paining or do you have back pain as well?
2. Have you tried any medications for the pain?
3. Do you have any trouble bending forwards or sideways?
4. Can you please type out the x ray report?
5. Do you have any other health issues apart from Shingles which you are undergoing treatment for?

Muscle/Skin pain not bone i.e Spine area. Towards evening I am inclined to stoop to my right side as this is the side affected - it helps to ease the pain. i am on no treatment for any medical condition. Thank you. Regards XXXX

Thanks for getting back, I am sorry for the short delay in my reply I was held up at my hospital

From the history you have given me it looks like you are suffering from fibromyalgia. However fibromyalgia is a diagnosis of exclusion. You need not approach an orthopedician for this issue, I recommend that you approach a general physician first.
Here is what I suggest you to do

1. Get CT spine of lumbo sacral region. X ray of the spine might not be enough
2. Send for routine blood tests like complete blood count, thyroid proflie, total cpk, random blood sugars
3. If everything comes back as normal then I would recommend a trial with pain killers. The best pain killer would be aceclofenac with a muscle relaxant. You can take it twice daily for not more than 3 days. If the pain does not subside then the only way to tackle this pain is to go for aquatic graded exercise therapy with the help of a physiotherapist
4. Some patients have also improved with anti depressant agents like duloxetine.
