default
Posted on Fri, 19 Feb 2016
Question: I suffered twice from renal tuberculosis & just completed the medication for 2 years. In 2006 & in 2013. Now suffering from prostate enlargement resulting into delay in urination. Some times I experience semen passing before or after urination. Wanted to know can it be due to masterbatation? Doctors had prescribed me Silodol 8 mg once in a day. Will it impact my sexual health. How can I get complete solution for problems like TB, urine burning, semen before or after urination etc.

Brief Answer:
Overall consideration of clinical picture and tests parameters

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Male/*37 - got treated for renal TB in 2006 and 2013 - Medication for prostate enlargement for a month - delay in urination - semen passing before or after urination - Silodol 8 mg once daily - Wants to know whether due to masturbation, sexual health and complete solution...

The semen coming before and after Urination can be in fact the prostatic secretion. It is better to get this tested for culture and sensitivity for the routine bacteria and tuberculosis.
Now-a-days we have the Genexpert machine which can detect the TB RNA and tell also about the sensitivity.
Semen coming at other times than ejaculation is not due to masturbation, you may please continue as it reduces the congestion of the prostate.

Silodol may not affect the sexual function much.

The following tests help to know the progress to some extent:
IVP: intravenous pyelography
Renal scan
CT scan for genitourinary system

Clinical evaluation and review regularly:
Gain in weight and appetite
Feeling good and better than before
Improved general condition.
Reduction or absence of symptoms
proper response to the treatment.

All these factors are to be considered together to decide the prognosis of the disease.

I hope this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication.

Hello Dr Chandrakant,
Thanks for the response. I really need your guidance on below points. Will try to follow your advise for testing.
1) Burning of urethra from tip to almost mid of urethra during urination or just if got feeling of urination.
2) Experiencing urethral burning before intercourse & after intercourse while urination.
3) Need to know the terminology in your response "reduces the congestion of the prostate."
4) Potential causes of left testicular pain - Had scrotum ultrasound & observed no special findings. Okay.
5) As per ultrasound report of scrotum prostate size is 24 CC & written okay then what could be causes of feeling pressure during urination. If prostate is okay then whether Silodal will work & how. Had one capsule yesterday & feeling relax today. (Feeling relax while urination, comfortable during a day but little dizziness.)
6) I am under treatment from well known urologist but need your guidance as sometimes I feel little discomfort near brain while semen is coming out during intercourse.
7) Lastly, is there any confirm treatment for renal TB or can you suggest some precautions to ensure no repetition of TB attack as I suffered twice from 2006.

Thanks for your descriptive guidance.

Brief Answer:
As discussed

Detailed Answer:
Hi.
Thanks for your further queries:
Burning sensation of urethra at the times as you have mentioned can be due a local cause like urethritis. This can be well visualized by Cyst-urethroscopy.
Reduce the congestion of the prostate means the internal swelling of the Prostate. This does not mean that the prostate is enlarged.

Potential causes of left testicular pain with normal ultrasound findings are: nerve pinching supplying the testis area, referred pain from the left ureter or prostate and bladder area.
Ultrasound shows prostate to be 24 cc, OK - feeling pressure during urination can be due to enlarged median lobe which is usually not seen during ultrasonography and on palpation during per-rectal examination, cystitis, urethritis, stricture urethra and so on.
Silodal works by relaxing muscles in the prostate and bladder. Dizziness is a common side effect of Silodal.
Nice to know that you are under a treatment of a well known Urologist. Discomfort near brain while semen is coming out can be due to psychological aspect related to this process.
You already have taken the best treatment for renal TB.
Maintaining a good balanced diet and exercises should help you to boost your immunity. A good immunity will not allow it to repeat.

I hope this answer along with the previous one helps you.

Hello Dr Chandrakant,
Thanks for giving me an guidance for understanding better.
How can I overcome the internal swelling of prostate & what could be the causes.
Is it possible at the edge of 37 Yrs.
I do not have any pain in or near to bladder. Yes surgeon diagnosed swelling on prostate by rectal examination & I too experienced little pain while seating. Hence undergone medication of 15 days & recovered. Left testicular pain is on off and not usual. One observation is during pain left testicle gets hard but after few hours or minutes gets relaxed automatically.
How can one detect enlarged median lobe? What can be done to overcome it.

Brief Answer:
As explained.

Detailed Answer:
Hello.
You have already been given a good medicine, that is silodol.
With the history of renal tuberculosis , this is possible at the age of 37.
This is prostatitis, Enlarged median lobe is seen on ultrasonography, and just enumerated as one of the causes.
It is good that you received medication and recovered well.
During pain the testis may appear tighter usually due to muscles getting taught and pulling testis up as a normal protection mechanism that the body has.

So far so good.
As said earlier you need to boost up the immunity.
All the best.
