What Causes Recurring Spotting With History Of Polyps?

Question: Dear Doctor,

How are you? I am writing to you after a two month gap, which means for two months I didn't have any issues. However, strangely enough, today I saw spotting again... :(...Uff...I hate it.

I didn't run to the doctor this time ...although I am concerned. Will take an appointment tomorrow though.

Anyhow, just want to let you know that the first day of my previous period was 6 Mar 15. And as you know, in Nov. 2015 I went through D&C wherein a uterine polyp was diagnosed.

Would like to know what your thoughts about the spotting. Your inputs would definitely help.

Thanks for writing to us with your health concern.
I remember you of course, and your history too.
Well, polyps DO recur, but I would not jump to such a hasty conclusion.
You have just seen spotting today, it could be mid cycle peri ovulatory spotting too.
Let me know how it goes on from here, if you have the kind of spotting pattern that you had previously.
Let us keep a track of it, and then decide if evaluation is necessary.
To be on the safer side, you can book an appointment for an internal exam, a Pap smear, and an ultrasound scan.

Dear Doctor,

Thank you for your reply. Today is day 2. I can see that my spotting is turning out into a light bleeding. I had used a panty liner and there was light bleeding. Now I have worn a normal pad. I have to see during the course of the day if it'll become like a normal period.

However, it is very, very strange to have a period this early. I did not have intercourse last month and so there is no chance of being pregnant. I did change the dosage of my thyroid medicine last week. Instead of taking 75microgram, I took 50 microgram. From today onwards I went back to 75microgram. I do not know if change in thyroid medicine can have an effect.

I have an appointment today in the afternoon but due to lot of pressure at work, I am not sure if I can go. Please advice doctor, if with these symptoms do you advice me to go for the appointment today?

Dear Doctor,

My last period was on 6 Mar 15.
I am expecting another around end of next week.
I had basically finished my thyroid medicine and I had only the 50 microgram. So I just continued with that till getting the 75 microgram.
I did have irregular spotting and bleeding many years ago when my thyroid had started, that is around 5 years back.
I can go today also, but only due to work reasons I was contemplating. If I don't go today I can try to go in another 2-3 days.
Doctor, I also wanted to just let you know that in 2013 my right ovary was removed as it had a borderline tumor.
Your inputs would definitely help me.

Well, this could be an earlier period, that you are having.
Yes, shifting thyroid medication dosages can do that in all likelihood.
NEVER titrate your thyroid medication dosages by yourself.
Also, get the levels checked every 6 weeks atleast, to know if proper dosage is being taken.
See, basically the point of the appointment is to see if the polyp has come back.
So if you are bleeding significantly, an internal exam would not be possible.
Also, you are busy today.
So why not book an appointment after a few days when the bleeding has stopped, or atleast come down to mild spotting ?
There is no real emergency for a check up right away.
Keeping in mind the history of the borderline ovarian tumour, one should never neglect any erratic vaginal bleeding.
I think this has happened because of the alteration in thyroid dosages, so start taking the correct dosage.
Wait for 3 - 4 days to see if this bleeding stops or spotting persists.
And then, irrespective of the outcome, have a checkup and ultrasound scan as soon as bleeding stops or lightens.
Also , when was your last Pap smear ?
If it was more than 6 months ago, please get that done too.

Dear Doctor,

Thank you for your reply. Yes, I would cancel today's appointment as I am having light bleeding. Will take the appointment when the bleeding reduces.

I didn't do any PAP smear in a very long time now. Maybe around 2 years. So I will definitely ask for one to be done.

With regards to the borderline tumor, I am due for the scan next month. However, my previous scans which were done in Dec. were all clean and my CA125 was 5. So I do hope and pray that this vaginal bleeding will not be associated to the borderline.

Will let you know about how the bleeding goes.

A Pap smear also is done when the bleeding is gone.
So makes sense to postpone the appointment.
I do not think the bleeding is associated with the ovarian issue.
If at all, maybe the polyp.
And not even that, I think it maybe due to the thyroid trouble.

Well, thyroid disorders can cause irregular cycles.
So this could be a period that is earlier than usual.
In that case, you can expect to bleed for as many days as you would, during a normal period.
Also, thyroid problems can cause mild mid cycle spotting too, so actually there is no way to predict how long you would bleed.
We can just go along with the days and see how it proceeds.

Dear Doctor,

Just wanted to drop in an update about my bleeding.

- 23rd afternoon: spotting started
- 24th: developed into bleeding like a period bleeding.
- 25th: still bleeding like a period bleeding, however already reducing.

My impression is that this is a period bleeding because I can see the usual light uterine tissue discharge which is normally associated with period blood. also, I had slight cramps which are again associated with periods. However, this period is definitely a lighter one in comparison to my usual period.

Will keep you posted.
