What Causes Persistent And Severe Abdominal Pain In A 60 Year Old Woman?

Question: Sir,
My mother, 60yrs having acute abdominal pain(severe) in lower,upper and right region for 20 days now.
She had some complain with the belly pain and low appetite for 4-5 months but it was not so acute but now the problem is severe.
Earlier(5 months ago)i contacted gastroenterologist and that time
1) endoscopy done with no such considerable findings
2) USG of upper abdomain done with no such considerable findings
What may be the reason for such acute pain?
It is worthy to mention that when this pain happens, she feels drowsy(eyes feel heavy).
Please suggest.

Please give additional details:
- Is there associated nausea, vomiting, bloating, loose stools or constipation?
- Any associated medical problems like diabetes or hypothyroidism?
- Fever, referred pain to back ?
- Any consultations or investigations done within these 20 days as she is suffering so much?
- Any other information that you would like to share.

This will help us to get to the point.

Thank you sir.
1)Yes she has back pain complain.
2)Constipation is my mother's life partner but occasionally she has loose stools-noticed for last 3 days(once or twice)
3)After around 3 hours of launch, the pain becomes considerable and sustains until at night she takes "Ultracet" medicine amd in the morning no such pain(generally she does not take any considerable food in the morning).
4) consulted gastroenterologist---suggested for colonoscopy(but it will take time).
I am feared of something emergency!!

Thanks for your feedback.
Noted the history of back pain, constipation, increase in pain after lunch till night for which she takes Ultracet and no pain in the morning, advised colonoscopy.

I would advise you the following:
CT scan of the abdomen as it is superior to ultrasonography.
Mild laxative on regular basis as clearance of the bowel is very important.
Colonoscopy as advised as one can see the whole large intestine, do biopsy if required and can have definite diagnosis.

Emergency situation if there are other symptoms or signs as described above like bloating, intractable vomiting, bloating meaning distension, and obstipation meaning unable to pass gas and stool.

The clinical evaluation as has been done the Gastroenterologist and investigations can give a proper diagnosis and the scope for proper curative treatment.
