Brief Answer:
as explained in details.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Male/21 - a little pain in testicle - achy - goes away with medicine - back pain in lower region - does not hurt always - comes and goes - wants to know the possibility and whether to seek medical attention in the morning...

Please give additional information:
Which side testicle and back has the pain?
Any urinary symptoms?
Any other symptoms?

The commonest causes of such a pain as you have described can be due to the following reasons:
- Ureteric stone which is not much obstructing the urinary flow.
- Urinary tract infection.
- Testicular problem like mild epididymo-orchitis

I would advise you the following:
- Drink plenty of water to keep the urinary tract flushed.
- Scrotal support.
- Send the morning sample of urine for routine, microscopy, culture and sensitivity
- Tests of blood.
- Ultrasonography of the whole abdomen, particularly for genito-urinary system and High resolution ultrasound with color doppler for the scrotum to rule out any pathology.
- Clinical evaluation and examination by the Doctor, investigations, proper management and reference to the Specialist if needed as per the final diagnosis made.

I hope, this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication.

Could that be the cause of the pain in testicles? Or could that be a more serious issues?

Brief Answer:
May not be a serious issue considering your age.

Detailed Answer:
Back muscle pains do not cause pain in testicle.
It may not be a serious issue.
Pain in testicle should be considered more important and diagnosed as early as possible.
Since you are 21, the pains in back may not be serious unless there is history of some direct or indirect trauma like accident, problem while playing or so. Is there any history like this?

Not that i know of, i mean my mother has a back issues but nothing serious....im not as worried about my back as the other issue... there is no swelling or anything and everything seems to be working correctly! I guess it feels like a knot sorta, its hard to descibe the pain in my testicles because its not constant or anything... there is no family history of testicluar cancer in my family. If i desrcibed the pain it would be like pressure/knot/throbbing but like i said it comes and goes. More so when i move around
