Hello,
I have studied your case and most of the reports are normal. Pain in neck along with shoulder pain is caused by disc herniation in the neck. It leads to compression of the nerve of shoulder and it causes pain. I have following suggestion for patients with cervical spondylitis.
1. Get MRI to confirm how severe is nerve compression. According to that we can plan further management.
2. Intermittent cervical traction exercise will help. Also neck muscle strengthening exercises is useful.
3. Avoid keeping neck in same position for long time and also do not use very big pillow.
4. Take Pregabalin and Methylcobal for shoulder pain. Muscle relaxant like Myoril or Myospaz is helpful for neck pain.

Number 2 means 2nd report of your attachments?
Physiotherapy is helpful in cervical spondylitis. It is given in two form.
1. In cervical traction, weight is applied to neck and gentle pulling helps in decompression of the nerves and it causes pain relief. Weight should be increased slowly and should not be more then 4 kg.
2.Neck muscle strengthening exercises. It improves muscle power of neck muscles and it causes better support to cervical spine, and it decreases neck pain.
